Claim Form Required In Carl’S Jr. Settlement

People who received hepatitis A immune globulin shots after eating at Carl’s Jr. restaurant in downtown Spokane must file a claim form in order to collect their money.

Restaurant officials last month agreed to pay for the shots for some 1,400 people who ate there earlier this year.

Those people ate at the restaurant around the time it was discovered an employee there had the virus but did not have the disease.

In order to collect a $200 settlement, each of those people who received shots must dial 1-800-769-9336, obtain a claim form and file it by April 2.
